Hospitals are increasingly requiring 2 years of case logs for new applicants

These can generally be obtained from your billing office or from the Medical Staff or Medical Records office at your current hospital. Most training program coordinators can also provide these for current residents and fellows. These are best provided in summary form (detailed case logs can be too cumbersome). An example of a summary case log report is included in the EXAMPLES section of this FAQ.

Case logs may be requested from HIM (Health Information Management) department, Medical records, billing, Human Resources or medical staff office.

Case logs may be requested from billing department, human resources or office/practice manager.

 

Q: Where do I request case logs/procedure logs from my physician management group/locums agency?

 Case logs may be requested from your physician management group through the billing department or on boarding coordinator. Your previous locums company is a long shot, but you can request them from the credentialing specialist.

 

Q: My hospital/surgery center does not keep track of my cases /procedures I work under a surgeon?
